How to fill in this form:


1. Include Customer #
2. Included Company Name
3. Include Invoice Number
4. Include Email Address
5. Include Item Number(s), Item Description(s), Quantity
6. Choose whether the item(s) is Damaged or Defective
a. If Damaged:
i. Please save the shipping carton the item was delivered in. Claims without cartons may be denied, since the carrier may request an inspection of the box and the carton MUST be available for inspection.
ii. Claims must be made within 3 business days of signed receipt of the goods.
iii. Photos must be provided (including outer and inner cartons)
b. If Defective:
i. Claims must be made within 3 business days of signed receipt of the goods.
ii. Photos must be provided
iii. In the event a “consumer” is missing pieces: Most items will have a customer service number enclosed where the consumer can obtain the pieces directly
